“Real time”: Example:
The patient’s initial assessment includes a plan of care that is in sync with the providers (physicians) treatment plan for the patient. A monitoring schedule is initiated, the patient is walked through the use of the device, confirmation of understanding is obtained, and the patient sends their data (readings) as prescribed. In addition, the patient is encouraged to send readings anytime they feel like there is a change in their body or when and if they have a concern. The data is sent within 3 sec. of the patient taking the reading. This is real time data.

The action plan for the patient is to call 911 in the event of an emergency and to seek urgent care in the event of an urgent concern. There is an internal procedure to manage emergencies should they occur within business hours and during active communication. The consents for participation include this information.
During non-business days and non-business hours, data is systematically time stamped & stored. Biometric values that are outside a patient’s baseline or set parameters are automatically flagged for clear view of the care manager. The care manager follows up the next day within business hours. Flagged data is a priority.
